Evolve Restorative Center has an exceptional opportunity for a qualified Physiatrist

to join our multi-disciplinary pain management and restorative care practice in

Santa Rosa, California. Our practice specializes in the management and treatment

of chronic pain with a focus on interventional modalities.

Our state-of-the-art practice has multiple clinical office locations including the flag-

ship facility in Santa Rosa. Located in the North Bay Area, we offer a beautiful and

culturally rich neighborhood in California Wine Country with excellent public and

private schools, theater and arts, parks and recreational activities. This position will

be full time, in the Santa Rosa location. Monday through Friday office hours, no

traditional call coverage or weekend hours.

Requirements

Medical degree (MD or DO) from an accredited institution with a focus on physical

medicine and rehabilitation. Board certification in Physical Medicine and

Rehabilitation is preferred. Autonomous practice experience with interpreting EMG

and nerve conduction tests, diagnosing neuromuscular conditions, and providing

treatment options to patients is essential. Must possess strong communication

skills and the ability to work effectively within a multidisciplinary team. Experience

with the EPIC medical records system and Cadwell EMG equipment is beneficial.

Responsibilities

1. Conduct thorough evaluations of patients to assess their physical condition.

2. Perform EMG and other nerve studies as necessary.

3. Develop and implement personalized care plans to address specific

musculoskeletal conditions.

4. Perform procedures as necessary including therapeutic and diagnostic

injections

5. Monitor patient progress and modify treatment plans as necessary to

achieve optimal outcomes.

6. Provide education and support to patients and their families regarding

treatments, testing and rehabilitation processes.

7. Maintain accurate medical records, including coding for procedures and

treatments.

8. Stay updated on advancements in the field of physical medicine and

rehabilitation.

Immediate opening for a Certified Nurse Practitioner or Physician Assistant in a nationally recognized, multidisciplinary pain medicine practice located in one of our North SF Bay Area locations. Job Requirements:

• Licensed Nurse Practitioner or Physician Assistant able to practice in the state of CA.

• Advanced nursing knowledge and clinical skills through an appropriate educational

program.

• Current DEA License.

• Works collaboratively with clinical and administrative staff.

• Compassionate care and professional demeanor.

• Clear communicator who fosters positive relationships with patients and colleagues.

• Excellent computer and organizational skills.

• Complies with federal, state, and local legal and professional requirements to acquire or

maintain licensure.

• Maintains professional and technical knowledge with continued medical education and

self-assessment.

• Meets all the requirements of the Medicare and Medicaid plans to be successfully

credentialed as a provider for these programs.

• Experience in a specialty medical practice, including pain medicine, neurosurgery, or

orthopedics, would be helpful.

• Familiarity with Wellness based clinical initiatives is also a plus.
